This website, seeifyouqualify.care (the "Site"), is operated by Mindcry Inc. ("we," "us," or "our"), located at 4134 Lake Conway Woods, Orlando, FL 32812.

We are a private insurance marketing organization. We are not an insurance company, and we are not affiliated with, endorsed by, or connected to Medicare, Medicaid, the Department of Veterans Affairs, or any other government agency. We connect consumers who request information with licensed insurance agents and agencies.

2. Information we collect

Information you give us

When you complete the eligibility form on this Site, we collect:

• Your name
• Your phone number
• Your email address, if you provide one
• Your state of residence
• Your answers to the eligibility questions (who the coverage is for, age range, whether help with daily activities is currently needed, and, on our veterans page, the relationship to a veteran)
• The date and time you submitted the form, and the version of the consent language you agreed to

Information collected automatically

• The web address of the page you submitted from, and any campaign tracking parameters in that address (for example utm_source, utm_medium, utm_campaign, utm_content and fbclid), which tell us which advertisement or link brought you here
• Standard technical information your browser sends, such as IP address, browser type and device type
• Information collected by cookies and advertising pixels — see Cookies and tracking

What we do not collect

We do not ask for and do not knowingly collect your Social Security number, financial account numbers, medical records, or detailed health history through this Site. A licensed agent may request additional information later, directly from you, as part of an insurance application — that process is governed by the agent's and the insurance company's own privacy practices.

3. How we use your information

We use the information we collect to:

• Connect you with a licensed insurance agent or agency who can answer your questions and confirm what products are available in your state
• Contact you about the request you submitted
• Keep a record of the consent you provided, as required by law
• Measure which advertisements and pages are effective
• Operate, maintain and improve the Site
• Comply with legal obligations, and detect and prevent fraud or abuse

4. Who we share it with

We share the information you submit with:

• Licensed insurance agents and agencies. This is the purpose of the form. The agent who contacts you may be employed by or contracted with us, or may be an independent licensed agent or agency we work with.
• Service providers. Companies that host this Site, deliver our form submissions, store our records, send email or text messages, or provide analytics on our behalf. They may use your information only to perform services for us.
• Advertising platforms. If you arrived from an advertisement, we may report that a form submission occurred so the platform can measure advertising performance. See Cookies and tracking.
• When required by law. To comply with a subpoena, court order, regulatory request or other legal obligation, or to protect our rights, safety, or the rights and safety of others.
• In a business transfer. If our business is sold, merged or reorganized, information may be transferred as part of that transaction.

We do not sell your personal information to unrelated third parties for their own independent marketing purposes. Sharing your information with licensed agents in order to respond to your request is not a sale for that purpose. 5. Your consent to be contacted

When you submit the eligibility form, you expressly consent to be contacted at the phone number you provide about insurance products — including by automatic telephone dialing system, artificial or prerecorded voice, and SMS text message — by us and by the licensed insurance agents and agencies we work with, even if that number is on a federal or state Do-Not-Call list.

Consent is not a condition of purchasing any product or service. Message and data rates may apply. Message frequency varies.

We keep a record of your consent, including the date and time of submission and the exact consent language shown to you.

To stop text messages, reply STOP to any message. To stop calls, tell the caller to place you on their internal do-not-call list, or contact us using the details in How to reach us.

6. Cookies and tracking

This Site may use cookies and similar technologies, including advertising pixels provided by third parties such as Meta (Facebook and Instagram) and Google. These help us understand how visitors use the Site, measure the performance of our advertising, and show advertisements to people who may be interested in our services.

Most browsers let you refuse or delete cookies through their settings. Blocking cookies may affect how parts of the Site work. Some browsers also offer a Global Privacy Control (GPC) signal, which we honor where required by law.

7. How long we keep information

We use reasonable administrative, technical and physical safeguards designed to protect the information we collect. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

Regardless of where you live, you may ask us to:

• Tell you what personal information we hold about you
• Correct information that is inaccurate
• Delete your information
• Stop contacting you

10. State privacy rights

Residents of certain states — including California, Colorado, Connecticut, Virginia, Utah, Texas, Oregon, Montana and others — have additional rights under state privacy laws, which may include the right to access, correct, delete, or obtain a portable copy of personal information, the right to opt out of targeted advertising and of the sale or sharing of personal information, and the right to appeal a denied request.

To exercise these rights, use our Do Not Sell or Share page or contact us directly. An authorized agent may submit a request on your behalf with proof of authorization.

California residents: in the preceding 12 months we have collected the categories of personal information described in Information we collect — identifiers, commercial information, internet activity information, and inferences drawn from them — for the business purposes described in How we use your information. We disclose those categories to the recipients listed in Who we share it with. We do not sell personal information, and we do not knowingly sell or share the personal information of consumers under 16 years of age.

11. Children's privacy

This Site is intended for adults. The insurance products described are issued to adults only. We do not knowingly collect personal information from anyone under 18. If you believe a child has submitted information to us, contact us and we will delete it.

12. Changes to this policy

We may update this Privacy Policy from time to time. When we do, we will revise the "Last updated" date at the top of this page. Material changes will be posted on this page.
